Coast Guard Special Needs Program 2015 enrollment deadline approaching



The Coast Guard Office of Work-Life is reminding all active duty members and selected reserve members on orders for 181 days or more who have dependent family members diagnosed with medical, psychological, physical, or educational special needs to ensure all enrollment documentation for the Special Needs Program be updated by Sept. 30, 2015, or be administratively closed on Oct. 1, 2015.

The Special Needs Program identifies and supports the special needs of a Coast Guard family while allowing the active duty member to maintain the maximum assignment flexibility.

Active duty members must enroll in the Special Needs Program if they have family members who meet enrollment criteria. Enrollment updates for each family member enrolled in the program are due every three years with the following exceptions:

• A change in medical or mental health related diagnosis, or a new diagnosis develops.
• A change in early intervention or special education needs.
• If an update is due at the time of a projected rotation date, it must be initiated at least nine months prior to that date.

The Family Member Medical Summary, DD Form 2792, and Special Education/Early Intervention Summary, DD Form 2792-1, should be completed by the family member’s medical provider or school personnel and submitted to the member’s servicing Health, Safety, and Work-Life Family Resource Specialist.

To facilitate assignment coordination and allow proper access to medical services by family members with special needs, a Coast Guard senior medical executive must assign all special needs cases.
